What are the benefits of FFS Rhinoplasty?
For many trans women, FFS Rhinoplasty can enhance the mental health-related quality of life. How long after surgery will I need to wait to see the final results of my Rhinoplasty?
While recovery is generally just a few weeks, final results of Rhinoplasty surgery will not be final until 8-12 months following surgery.
Can I combine Rhinoplasty with another surgery?
Yes, you can combine Rhinoplasty with Transgender Breast Augmentation, plus Body Feminization (liposuction, limit of 1000cc of fat removal) in the same operative event.

Should I choose silicone or saline breast implants?
There are pros and cons to each, Silicone implants feel more natural in very thin breasted patients, Saline implants can be more noticeable to the touch and have some cosmetic issues as rippling. However, Silicone implants are pre-filled and the larger size is 800cc, Saline implants can be over inflated to give you a more voluptuous look, Saline implants can also be made larger at a later date by just adding Saline. Dr. Alexander will discuss this with you during your consultation and will offer you his professional recommendation based on your goals.
Will I only be able to get small implants because I don’t have “real” breast tissue?
No. Even with limited breast tissue it is possible for Dr. Alexander to insert quite large implants and, if necessary, add more Saline at a later date when the breast tissue has stretched out
Will I have a lot of scarring from this procedure?
Dr. Alexander usually inserts implants through a small incision around the areola. This strategic placement ensures a small, well-concealed scar.
Does MTF Transgender Breast Augmentation require an overnight stay in a hospital?
No, MTF Transgender Breast Augmentation is performed as an outpatient surgery. After your surgery, you will spend some time in the recovery area and then you’ll be discharged. You will need to stay in South Florida from 5-7 days after top surgery, for a follow-up appointment with Dr. Alexander.
Will I experience a change of sensation in my breasts following surgery?
You may also have a change in the sensation of your breasts immediately post-op, but this usually subsides quickly.
Can I combine Breast Augmentation with Rhinoplasty?
Yes, you can combine Breast Augmentation with Rhinoplasty, this is actually a very common combination for Dr. Alexander’s MTF Transgender patients.
Can I combine Breast Augmentation with Liposuction?
Yes, you can combine Breast Augmentation plus Liposuction in the same operative event.
Will Dr. Alexander provide me with a notarize letter for my legal name change process?

What is Body Feminization?
Body Feminization, also called Body Contouring and Body Sculpting, refers to the use of surgical techniques that add feminine curves to your figure, such as Liposuction and Tummy Tuck.

Brazilian Butt Lift / Hourglass Lipo-Fat Transfer

I am very skinny, would I still be able to have the BBL / HL-FT surgery?
If you don’t have enough donor fat, Dr. Alexander will ask you to gain weight (5-8 Lbs.) before having your Transgender Brazilian Butt Lift surgery.
Can I combine BBL / HL-FT procedure with Transgender Breast Augmentation or Rhinoplasty?
No. The BBL and the HL-FT CANNOT be combined with ANY other procedure.
Do I need to take time off work/school after Transgender BBL / HL-FT?
Yes, you will need to take 10-15 days off work/school. After your Transgender BBL / HL-FT procedure you will not be able to sit or sleep on your back / side for 10-15 days after surgery (the longer the better).
What’s the difference between the Brazilian Butt Lift and Buttock Augmentation?
The Brazilian Butt Lift is a “closed procedure,” whereas traditional butt augmentation surgery is an “open procedure,” with larger incisions and either saline or silicone implants. Closed procedures have a shorter recovery period, and less risk of infection, and no risk of implant rejection (since no implants are used.)
Without butt implants, how exactly does the Brazilian Butt Lift work?
This procedure creates a fuller, curvier, and more lifted backside. Instead of implants, Dr. Alexander utilizes liposuction to remove fat from “problem areas” on your body, such as your flanks, thighs, back, and/or abdomen, then strategically injects the purified fat into the upper buttocks to create an enhanced, natural-looking curve.

What types of Top Surgery does Dr. Alexander offer?
Dr. Alexander is able to perform top surgery using any of the surgery methods: Double Incision (DI) with nipple grafts, T-Anchor, Peri-areolar, and Keyhole. To learn more about how these methods differ, please see the FTM Top Surgery page.
How do I choose the type of Top Surgery procedure that’s right for me?
Dr. Alexander will explain the differences between each technique and offer his professional recommendation for the procedure that best suits your body composition and goals.
Do I need to take time off work/school after Top Surgery?
Yes, you will need to take 1-2 weeks off school or a desk job. For more physical jobs, and sports, you will need to take 4-6 weeks off. Too much activity following surgery can negatively affect healing and results.
Does FTM Top Surgery require an overnight stay in a hospital?
No, Top Surgery is performed as an outpatient surgery. After your surgery, you will spend some time in the recovery area and then you’ll be discharged. You will need to stay in South Florida for at least a week after top surgery, for a follow-up appointment with Dr. Alexander.
Can I get Pectoral Implants when I get my Top Surgery?
No, you must wait at least 3 months after Top Surgery before getting Pectoral Implants.
Can I get Body Masculinization in conjunction with Top Surgery?
Yes, but there is a limit of 1000cc of fat removal.
How much pain will I be in after Top Surgery?
You will feel some discomfort after surgery, but the pain will be less than moderate. Dr. Alexander will prescribe pain medication. For many, regular Tylenol is enough. If you have drains from your surgery, you may experience some pain where they are inserted. Icing provides good relief.
When can I go back to the gym after Top Surgery?
If you are a bodybuilder, wait 6-8 weeks minimum before returning to regular workouts. More strenuous chest workouts should be avoided for 2-3 months. Going back to the gym too soon can stretch your scars and make them more noticeable.
What can I do to help my Top Surgery scars heal well?
Dr. Alexander will provide you with post-operative instructions with regards to minimizing scars. Other ways you can minimize scarring: wear a compression vest after surgery for 4-6 weeks, avoid reaching up over and behind your head for 4-6 weeks, use a silicone scar gel or sheeting, keep your scars out of the sun for one year (wear a T-shirt or apply a strong sun block.)
Do I need to have a mammogram prior to Top Surgery?
Yes, but only if you are 35 years of age or older. Dr. Alexander can provide a prescription for a mammogram.
Does Top Surgery remove all risk of breast cancer?
Top Surgery does not remove all breast tissue, so the risk of breast cancer, though greatly reduced, remains. However, mammograms will no longer be recommended following top surgery.
I’m 16 years old. Can I get Top Surgery?
If you are 16 to 17 years old Dr. Alexander requires you parents/legal guardian’s consent and a referral letter from your Gender Therapist, Psychiatrist or Psychologist. The recommended content of the referral letter must state you are psychologically ready and surgery is the next step in your therapy. Dr. Alexander also requires a Medical Clearance from your primary care physician.
Will Dr. Alexander provide me with a notarize letter for my legal name change process?
Yes, you need to let your patient coordinator know in advance how many letters you will need and for what agencies: Social Security, Court Order, Passport.
Do I need letters from my therapist to qualify for Top Surgery?
Yes, per WPATH standards Dr. Alexander requires that patients have referral letters. Please see the FTM Top Surgery page for details.
Do I need to be on testosterone to get Top Surgery?
No, you do not need to be on hormone therapy now, or at any point, to have Top Surgery.

What is Body Masculinization?
Body Masculinization, also called Body Contouring and Body Sculpting, uses Liposuction to remove fat from various parts of the body, creating a more masculine body contour. To help eliminate feminine curves, Dr. Alexander can remove fat from the hips and buttocks.
